Open "Give Us Feedback" in a new window

Open a new window when clicking the “Give Us Feedback” link, so we don’t lose our reference screen & work in ADMM.

Ideally, fill some details by default. For example, fill the Module with the value of the Module that was in use when the “Give Us Feedback” link was clicked (but allow to change it manually).
