Enhance MIG_REPORT With Created By/On

It would be helpful to have fields on the MIG_REPORT view to show the user that created the report, as well as the date it was created. This can be helpful to identify reports created by a user vs. a report created by the 'Create Reports' button. It also would benefit project leads to determine who is making reports and how often.
